Student Registration Information

Welcome to Preble Shawnee! We are excited to partner with FinalForms, an online forms and data management service, to make registering your student fast and easy.

Kindergarten – 12th Grade Registration

Step 1: Online Registration

  1. Access the FinalForms Login & Registration Page.

  2. Since your student is new to our district, be sure to select the “New to District” checkbox.

Step 2: Documentation & Next Steps

After completing your online registration, a district representative will contact you to schedule an appointment to collect necessary documentation, including:

  • Student’s Birth Certificate

  • Proof of Residency

  • Updated Immunization Records

Preschool Registration Information

Our state-licensed preschool program serves children ages 3–5, including those identified with special needs, following guidelines set by the Ohio Department of Education.

Using a developmentally appropriate, play-based curriculum, our integrated program fosters intellectual, social, physical, and emotional growth—allowing children of all skill levels to learn, interact, and build friendships together.

Preschool Eligibility Requirements:

  • Must turn 3 years old by August 1st.

  • Cannot be eligible for Kindergarten (5 years old by August 1st).

  • Must reside within the school district.

How to Apply

Preschool enrollment opens in April of each year. To join our waiting list, please fill out the interest form below:
